Contained in this section are Learning Log forms which you will need to complete for each workshop, coaching practice within workshops, and for your formal practice sessions with your nominated coachees. Personal Programme Objectives Form

You will have stated your objectives on your application form and discussed them in the interview process and these should be noted in this form. This Form will become a working document that you can continue to develop throughout the programme. You may want to add to or amend your objectives after the Pre-Programme one-to-one.

Workshop Preparation & Review Form

Before every workshop, participants are required to complete the Workshop Preparation & Review Form, outlining their personal goals and objectives for the day(s). This invites you to focus on what you would like to learn.

The Review section of the form is to be completed after every workshop. It invites you to record your learning from the coaching practice and from the day’s topic. The final question asks you to think about your coaching development as a result of the workshop. When completing the form, it might be useful to refer back to your personal programme objectives.

Feedback & Self-Evaluation Form

For every practice coaching session you hold (both in workshops and outside with practice coachees) a Feedback & Self-Evaluation form must be completed by you and the person you have been coaching.

Feedback Form

The Feedback Form can be used by a number of people. It can be used by your coachees (outside of the workshops) and by your observers or your programme leaders (inside the workshops) as a means of giving you written feedback on your coaching style. The form has two main sections:

  1. The Skills & Qualities section is completed by you, using selected headings from The Coaching Development Log, appropriate for your learning in that session. (Please read The Coaching Development Log section before completing.)
  2. The General Review section provides an opportunity for your coachee/observer to note down, in their opinion, what went well in the session and what did not work well. It also asks the coachee to think through about anything specific you may need to focus on in future to develop as a coach.

Self-Evaluation Form

The same form used for feedback can also be used for your own reflection and review on how you thought the coaching session went.

  1. Again, the Qualities section requires you to evaluate yourself against the qualities or skills you have chosen to work on in that particular coaching session.
  2. The General Review section provides an opportunity for you to record, in your opinion, what went well in the session and what did not work so well. It also asks if you think there is anything you need to learn or develop.


Finally you are asked to note the techniques, structures and models you employed during the session.

During the programme, some of your coaching will be recorded. We would recommend that you use this footage to aid you in completing self-evaluation forms and thinking about areas for further development.

Coaching Notes Form

As part of your development as a business coach, the keeping of professional records is essential. You are therefore required to complete a Coaching Notes Form after each practice coaching session with one of your practice coachees.

This helps to maintain a professional approach to coaching, provides a structure for capturing an accurate account of the session, and an opportunity for the coachee to reflect and stay focused.  

This form is also useful as an aide memoire to take forward agreed actions and to provide a bridge between one session and the next, making for a tight and focused set of sessions. You are asked to record the topics discussed, any key points raised, and actions to be taken.  

Your coachees may like copies of the meeting reports.  An explanation of this process can therefore form part of the initial contracting meeting.

Website & Book Review Form

As part of your portfolio requirement, you must complete three book reviews and one website review, completing a Book & Website Review Form for each. Please refer to the Appendix in your workbook for a list of recommended books and websites.
